Wound Care Nurse Practitioner/Physician Assistant - Wound CareSonora, CA
Full-Time or Part-Time | Weekday Day Shift | No Weekends
We are currently seeking a skilled and compassionate Wound Care NP/PA to provide specialized wound management services in collaboration with facility wound nurses and interdisciplinary care teams.
This role is ideal for a provider who values clinical autonomy, collaborative care, and delivering measurable improvements in patient outcomes.
The Wound Care NP/PA will evaluate and manage acute and chronic wounds, including:
Pressure injuries (all stages)
Diabetic foot ulcers
Venous stasis ulcers
Post-surgical wounds
Skin tears and rashes
Malignant wounds
You will use evidence-based practices to develop individualized treatment plans and monitor healing progress.
Key Responsibilities
Patient Assessment & Treatment Planning
Perform comprehensive wound assessments and physical exams
Develop and manage individualized care plans
Order and interpret diagnostics as needed
Prescribe medications and topical treatments
Advanced Wound Care
Perform conservative sharp debridement
Manage negative pressure wound therapy
Adjust care plans based on wound progression
Maintain thorough documentation and wound photography
Patient Education & Support
Educate patients and caregivers on wound care best practices
Provide guidance on pressure offloading and nutrition
Promote strategies to prevent recurrence
Collaboration & Compliance
Work closely with physicians, nurses, therapists, dietitians, and case managers
Participate in care coordination and case reviews
Ensure documentation meets CMS and regulatory standards
Qualifications
Required:
Master's degree from an accredited NP or PA program
Active, unrestricted California NP or PA license
National board certification
Active NPI and DEA (or eligibility)
Preferred
Experience in wound care, dermatology, or surgical nursing
Familiarity with electronic health record systems
Wound care certification (WCC, CWCN, CWS) preferred but not required to start
Compensation & Schedule
Base Salary + RVU Incentive
Annual Compensation Range:
$40,000 - $160,000 (based on schedule and productivity)
Day shift during the week
No weekend shifts
Flexible part-time or full-time options
