    Position Summary:

    Provide patient care that optimizes the use of medication and promotes health, wellness, and disease prevention.

    Essential Duties and Responsibilities include the following:

    All processes of care/Pharmacist Referrals:
    • Regimen review, ensuring safe medication administration, monitoring for desired therapeutic outcomes, identifying and avoiding medication errors and adverse drug events, etc.
    • Provides pharmacological information by answering questions and requests of health care professionals
    • Works collaboratively with Management staff in the medication management of patients transitioning from inpatient to outpatient or next level of care
    • Counseling patients on drug therapies
    • Reduction of hospitalizations and ED visits, avoid poly-pharmacy issues, drug interactions, assistance w/medication
    • Provides clinical and educational support to staff regarding pharmacy and medication issues
    Disease Management for Diabetes, COPD, CHF
    • Knowledge of chronic disease/conditions and provide treatment goals
    • To provide disease specific education to patients and improve patient participation in care
    • To educate health care workers and the community about the importance of preventive measures and medication compliance in disease state management
    Post Discharge Program:
    • Reviews all medications and ensure accuracy and effective functioning and manage all communication with physicians and patients and assist to resolve all patients within required timeframe
    • Involved in the education of patients about new medications and in the education of all patients about their discharge medications
    • Discover and awareness of discrepancies, and to provide a safe and effective process for obtaining, documenting, and communicating medications across a patient's continuum of care
    IDT Meetings:
    • Participate and maintain a significant role in multidisciplinary patient care rounds
    Outreach Programs/Events:
    • Participate in different activities of outreach program to talk about the importance of medication management
    Prior Authorization/Financial Assistance:
    • Provide support to patients and physicians when medication is recommended and prescribed but not covered by patient's health plan pharmacy benefit and lack the financial resources to pay for their medications and co pays
    ACO Program
    • Reviews medication therapy to advance patient care, focusing on various approaches
    • Working together with providers/physicians to improve the quality of health care services and reduce costs
    • Advise prescribers regarding the relative effectiveness and value of their drug treatment options
    • Responsible for the ongoing management of medication therapy and outcomes measurement
    The pay range for this position at commencement of employment is expected to be between $125,000 - $120,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

    The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

    Requirements

    Education and / or Experience:
    • Graduate of school of pharmacy with a Doctor of Pharmacy (Pharm.D.) degree.
    • Current licensure as a pharmacist in the State of California
    • Ability to exercise independent judgment and prioritize a diverse workload.
    • Outstanding organizational and communication skills.

    • Proficient understanding of common computer programs used in pharmacy practice, and the ability to type at least 30 words per minute is preferred.
    • Qualified in Pharmacy Services Guidelines for assigned service area is preferred.
