    Summary

    The Licensed Practical Nurse provides general nursing care to patients, families, and caregivers in the Medical Group practices. Assisting and advocating for the patient and families health in support of the patient-centered approach and delivery model of care are key responsibilities. Collaborating with physicians/providers, registered professional nurses, and multidisciplinary care team members is a crucial aspect of the role. The nurse follows the standards outlined for Licensed Practical Nurses and adheres to the mission, vision, and values of Mohawk Valley Health System.

    Main Responsibilities
    • Gather patient measurements and data
    • Record and report patient information
    • Prepare patients for examination and treatment
    • Assist with examinations and treatments
    • Perform tasks within supportive care framework under clinical direction
    • Support self-care and the established plan of care
    • Maintain equipment and a safe work environment
    • Stay updated on technical knowledge and educational workshops
    • Participate in quality improvement initiatives and communicate effectively within the healthcare team
    • Respond to patient requests and communicate to the appropriate healthcare provider
    Education/Experience

    Required:

    • Graduate of an accredited licensed practical nursing program

    Preferred:

    • Bachelors or Masters of Science in Nursing
    Licensure/Certification

    Required:

    • Current New York State licensure as a Licensed Practical Nurse
    • Basic Life Support Certification within 6 weeks of hiring
