    Job Description:
    • Primary operations expert for Caregiver guides demonstrates deep understanding of caregiver services, HIP platform and established processes.
    • Enhances informal caregivers participation in suitable programs and services.
    • Serves as a consultant/educator regarding healthcare inquiries focusing on health literacy obstacles.
    ,

    The role assists with transitions in member care settings when necessary and produces reports concerning program metrics associated with caregivers. Participates in community efforts including regional caregiver advisory committees and provides educational support for providers.

    This position collaborates internally with Pathways staff and Interdisciplinary Care Team (ICT). Requires valid driver’s license, automobile insurance and good driving record.
