    Summary

    This position assists members in their communities to access physical, oral, and behavioral health services by building trusting relationships.

    This enables the community resource specialist to serve as a liaison linking intermediary between the member, various services and the community, improving quality, and cultural competence for services being rendered.

    This improves capacity by increasing health literacy, knowledge, and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.

    Essential Duties


    Provides information, assistance, tools, and support to enable a member to make the best health care decisions for their unique personal situation.

    Works in the office making phone calls and occasionally makes visits to member's homes and/or multiple care settings, as needed.

    Completes all required documentation inclusive of electronic documentation in a timely manner in accordance with regulations and AllCare CCO and Medicare Advantage policies and procedures.

    Participates in the development of individual treatment plans with the member and their primary care team.

    Assists in removing barriers to care and individual self-management goals by bridging community, cultural, linguistic, educational, and other barriers to encourage self- care.

    On Call Responsibilities

    This position may have voluntary on- call responsibilities.

    Job Requirements

    Requires the use of personal vehicle for local travel (subject to mileage reimbursement).

    Education & Experience


    High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.

    College degree, preferred.

    Basic medical knowledge including ability to recognize social and medical risk factors, trauma symptoms and behaviors, and chronic disease conditions.

    Certificates, Licenses and/or Registrations

    Valid Oregon Driver's License and vehicle insurance.
    Completes the National Provider Identifier (NPI) registration within the first 30 days of hire.
    Current Basic Life Support (BLS) Certification or willingness to be certified within 90 days of hire.
    Current Mental Health First Aid Certification or willingness to be certified within 90 days of hire.
    Traditional Health Worker (THW) certification through OHA is required or to be obtained within two years of hire date. (Subject to pay increase upon Certification)

    The employee has exposure to communicable disease and body fluids.

    Because of exposure to patient records of all types, the highest standard of patient confidentiality and privacy as established by business policy and HIPAA requirements must be maintained.

    The employee must be able to work onsite for all scheduled shifts.
    The employee must be able to travel locally, as needed.
