    RESPONSIBILITIESAssists in scheduling, transporting, transferring, and lifting patients as required for rehabilitation servicesDocuments and communicates scheduling conflicts with participantsDistributes schedules and other information to appropriate facility members, as directedConducts maintenance program with patients to maintain their functional statusAssists the physical therapist (PT) and PT assistant with treatment, including ambulation, ROM and hot/cold packsAssists the occupational therapist (OT) and OT assistant with treatment, including activities for daily living (ADL) and feeding programsAssists the speech-language pathologist with treatment, including feeding groups, functional maintenance, and memory booksMaintain rehabilitative equipment, as neededMaintains general upkeep of therapy areas, including set-up and clean-up of rehabilitation area and suppliesRemains updated on medical and physical statuses for participantsAttends in-services and staff meetings, as directedEDUCATION/EXPERIENCEHigh school/GED requiredValid BLS certification required upon hireOne year experience working with elderly and frail populations requiredExperience in rehabilitation setting preferredADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)

    Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with working with elderly and frail populationsAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 50 lbs/weightAbility to stand for long periods of timeNeighborhood Healthcare offers a generous benefit plan that includes:
