    We are seeking home providers in Pottawattamie County, IA. Must know American Sign Language (ASL).

    Become a Host Home/Shared Living provider with Mosaic at Home and earn an average annual income of $80,000 (federally tax-free) plus room & board. You must reside in Iowa due to state funding.

    Mosaic has provided improved quality of life, greater stability, and increased community access for people with disabilities for nearly 40 years.

    Your Role as a Home Provider

    As a home provider, you will share your home with an individual with a disability and maintain a safe and healthy environment. Your responsibilities include being a mentor and assisting with daily living tasks as needed.

    Our Client Profile

    We have a female client in the Council Bluffs, IA community who is seeking a provider to share her home. She enjoys bingo, spending time with friends, and going shopping. The ideal provider would be familiar with American Sign Language.

    Key Responsibilities

    • Support with cooking, cleaning, laundry, budgeting, and decision-making
    • Skill acquisition and medication administration
    • Medical appointments and overall health support
    • Daily documentation via web-based case management application (THERAP)
    • Provide leisure experiences and opportunities for community inclusion
    • Compliance with state and agency policies

    Possible Additional Responsibilities

    • Assistance with bathing, toileting, feeding, and behavioral support
    • Support for medical protocols and complex physical/medical needs

    Requirements

    • Residency in the state where you wish to provide services
    • Transportation, valid driver's license, car insurance, and registration
    • Active home/renters insurance
    • Owning or renting your own home/apartment/condo/townhouse
    • Age 18 or older
    • Experience preferred but not required
    • Reliable computer and internet access
    • Completion of all state-mandated trainings
    • A safe and clean residence
