- Coordinates with other departments to create a comprehensive treatment plan for residents
- Communicates and establishes discharge and treatment goals, while coordinating with other agents/disciplines
- Advocates for residents, ensuring their rights granted under federal statues are protected
- Liaises between families and residents, and between residents and staff
- Tracks and manages mental health, well-being, cognitive loss of residents, using formal and informal processes
- Oversees room replacements/changes
- Maintains records and statistics on areas such as admissions, discharges and change statuses
- Interfaces with and trains staff on how to manage residents' behaviors/diagnoses
- Ensures all suspected abuse/mistreatment of residents or their families is reported to the proper authorities
- Assists staff and residents with miscellaneous needs
- Performs all other duties as assigned or directed

- Education: Bachelor's degree in Social Work/Services or related field (industry and state-specific)
- Experience: Must have two (2) to three (3) years of relevant work experience
- Computer Skills: Must be able to proficiently operate a computer, the Internet, Microsoft Office and other relevant software programs, and basic office equipment
- Certifications & Licenses: Licensed Social Worker (LSW) or Master of Social Work (MSW) preferred
- Other Requirements: Must have strong technical writing skills; Must have strong communication and interpersonal skills and be able to effectively interact with individuals from various professions, background, ethnicities, etc.; Must have strong counseling skills (including the ability to talk to others, good listening skills, interpretive skills with people who may not be able to communicate effectively); Must have a high degree of patience and empathy, especially with residents and families; Must have some experience with general maintenance; Must be able to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
