    Description

    CHM Mobile Care, LLC is seeking a qualified Nurse Practitioner to join our practice in Kentucky. Our focus is Transitional Care, seeing patients post-discharge from the hospital, skilled nursing, or inpatient rehab. The role involves seeing patients in-person and via telemedicine visits.

    In this role, you will see four to six patients per day in community home visits or assisted or independent living facilities or perform telehealth visits. For each patient, you will evaluate and assess the patient's condition, understand the treatment plan from the discharging physician, and provide direct patient care. You will also educate patients and their caregivers on appropriate measures to prevent infection, manage chronic conditions, and address lifestyle changes which may be necessary to positively impact the patient's health.

    To qualify, you will have:

    • An Active and unencumbered Nurse Practitioner License for the state of Kentucky
    • An Active BLS Certification
    • Experience in providing direct patient care
    • Reliable transportation, and internet service
    • Excellent communication and collaboration skills
    • Experience with Electronic Health Records systems
    • Availability to work one or more day per week
