- Evaluates clinical case management activities and conducts monitoring and evaluation of the Clinical Case Management Program using objective criteria.
- Provides a holistic approach to case management, case coordination, and discharge/disposition services.
- Organizes and updates internal and community resources to provide effective patient management, promote quality outcomes, and reduce costs.
- Review/analyzes medical records, and other associated documents to determine quality and appropriateness of clinical care requirements.
