- Providing case management activities to older frail adult clients
- Conducting in-depth intakes assessments covering psychological social physical environmental concerns for frail Medi-Cal-eligible older adults and their families
- Evaluating participants identifying potential limitations which impede independent living activating resources to allow participants to remain safely in their home setting
