    Description

    To coordinate and implement community health activities that support the process of identifying community health needs, selecting priorities and implementing evidenced-based programs to address these needs at one or more hospitals. The incumbent will be responsible for supporting the Community Health Needs Assessment cycle and then planning, implementing and tracking the results of specified community health programs. This person will actively represent the hospital in a variety of coalitions and partnerships in the community.

    Major Responsibilities:

    • Coordinates evidence-based community health programing that support needs identified at designated hospital sites.
      • Designs community health programs to address needs in collaboration with community partners.
      • Coordinates and implements specific tactics associated with these programs such as attending meetings, ordering supplies, gathering data, coordinating events and screenings and providing support to community partners.
      • Collects monitoring and outcome data associated with the various community health programs.
      • Identifies any problems that arise during implementation that may interfere with the overall success of the program objectives and/or the community partnership. Makes recommendations to Director for program improvements as needed
    • Contributes to community health leadership and planning at designated hospital sites.
      • Participates in site Community Health Council meetings and other site meetings that promote system alignment with community health strategies.
      • Collaborates with the director to implement data collection activities related to the Community Health Assessment to enable each hospital to measure community health needs and the impact of hospital interventions
      • Implements community programming to support population health improvement in areas of identified community need with emphasis on specific populations with health disparities.
      • Educates associates, management and hospital governance on issues impacting the health of the community and on the hospital's planned interventions to impact these issues.
      • Implement and monitor outcomes for service line initiatives and Advocate population health community based initiatives.
    • Implements activities to ensure accurate collection and summary of community benefit data for designated hospital sites.
      • Facilitates reporting of community benefit activity for the hospital to ensure accuracy and monitor on-going hospital community benefit reporting
      • Addresses training issues with various hospital departments as needed to ensure reporting compliance. Monitors accuracy of activities and provides feedback for improvement.
      • Attends Community Benefit Task Force meetings, to provide input on reporting tools, and annual audit meeting.
      • Acts as liaison between hospital associates and system Community Benefits Consultant
      • )Makes recommendations to Director and system Community Benefits Consultant to improve the Community Benefit reporting process and worksheets.
    • Manages strategic community relationships at designated hospital sites.
      • Establishes and maintains trusting relationships with community leaders/representatives, faith groups, community based organizations, political/government officials and non-Advocate health care organizations/hospitals as well as other Advocate hospitals to address community needs
      • Represents designated hospital sites in local coalitions with a focus on coalitions addressing community health needs.
      • Communicates site CHNA findings to community groups and solicit involvement in addressing identified needs
      • Makes recommendations to site and system leadership related to strategic partnerships opportunities engage community to address community health needs.
      • Supervises interns, volunteers and /or consultants as needed with input from regional director.


    Education/Experience Required:

    • Must have strong organizational and interpersonal skills as well as good writing and oral communication skills. Bachelor's degree especially in a health related field and 2-3 years of community health experience as a volunteer, intern or a paid professional. equivalent education & experience.


    Knowledge, Skills & Abilities Required:

    • Computer literacy in Microsoft programs including Excel, Publisher, Word and Access.


    Physical Requirements and Working Conditions:

    • Ability to travel within the metro community. Frequent need to attend off-site meetings Able to work on evenings or week-ends in community
    • Generally light
