- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Coordinate and monitor patient care throughout the healthcare continuum
- Collaborate with healthcare professionals to ensure the delivery of highquality care
- Educate patients and their families on disease management, treatment options, and available resources
- Advocate for patients' rights and ensure their privacy and confidentiality in accordance with HIPAA regulations
