    Job Description:

    TEAM SUPPORT

    • Orients new volunteers and provides ongoing education in the program to assist with every step and assist the Program Director to provide leadership and oversight for NCEF Pediatric Diabetes Prevention Program
    • Develops and maintains computerized databases to facilitate management and coordination of all services and contributes to communication and collaboration with interdisciplinary team members to best meet needs of patients enrolled in NCEF Program.
    • Conducts meetings with the team members that include a weekly education focus, announcements, records and facilitated patient care intervention discussion based on the progress of the patient.
    • Organizes and archives team education materials.
    • Assists Program Director to coordinate external supportive services for volunteers in maintaining self-care
    • Serves as a resource for team member concerns or support needs.

    PATIENT SUPPORT

    • Plays a critical role in this program by being our "boots on the ground" resource to communicate with community programs, promote our program to community pediatricians, identify children in need, work directly with families to provide them with iPads, so that they can successfully complete telemedicine visits with our UF team, coordinate in in-person RD visits with our subcontracted providers in Collier county for families who need in person teaching, and work with HADC to provide supplies to children and family in need.
    • Assists the Program Director to develop care plans to facilitate services that enhance patient and family adjustment to quantitatively or qualitatively life-limiting illnesses.
    • Introduces the NECEF program to new designated patients.
    • Regularly coordinates activities for patients/families (milestone events, unique long-term patient events, etc.) including management of logistics, catering, and community involvement.
    • Encourages patients to seek opportunities and build supportive relationships with like-minded patients and volunteers.

    ADMINISTRATION

    • Liaison and representative to the NCEF specific teams, interfacing directly with the clinical team, managing all involvement in clinics and community events; responsible for the scheduling of team schedules and attendance, call-in support for patients in immediate need.
    • Assists the Program Director in the following:
    • Recruitment, screening, interviewing and orientation of new team members; supervises OPS and volunteers.
    • Reviews EPIC charts and filters patient census and medical information into a team-member appropriate "need-to-know" format.
    • Records daily patient referrals from medical staff and consolidates information into task-lists for team members; manages pst files.
    • Manages Programs' documents featuring weekly team educational readings, announcements, and educational web links.
    • Ordering and maintaining inventory of program supplies
    • Record keeping of receipts, time logs, travel documentation and all other financial records.
    • Facilitates communication and collaboration with interdisciplinary team members to best meet the needs of patients enrolled in the Program.
    • Assists in grant writing, data collection, and promotional materials to support funding proposals and recognize donors.
    • Manages patient files including patient pictures, diagnoses, and team member psychosocial chart entries.

    PROGRAM PROMOTION AND EXPORTATION

    • The incumbent assists the Program Director with developing, coordinating, and participating in community activities through individual and group support that promotes optimal quality of life and health of these patients and their families.
    • Responsible for the social media and promotional materials and promotion of NCEF events.
    • Responsible for promotional materials of the NCEF program including brochures, pamphlets, and other printed materials.
    • Manages the NCEF website and Facebook page, ensuring all quality and privacy standards are met.
    • Initiates and facilitates community relations to support NCEF activities.
    • Provides services to promote and provide information about NCEF Program.
    • Identifies areas of potential research for program and outcome evaluation.
    • Collaborates with researchers at the University of Florida on developing program research.

    Expected Salary:
    $27,000 - $29,000 annually to align with part-time FTE


    Minimum Requirements:


    Bachelor's degree in in an appropriate area and two years of relevant experience; or an equivalent combination of education and experience.


    Preferred Qualifications:

    • The ability to make important connections in the community and perform home visits with families
    • Current Diabetes navigator experience and understanding of T1D and T2D
    • Experience working with pediatrics patient population
    • Being bilingual, fluent in Spanish
    • Good communication skills
    • Being flexible, a critical thinker with the ability to collaborate with different teams of individuals while maintaining a positive, can-do attitude
