- Conduct comprehensive patient assessments and develop individualized care plans.
- Deliver skilled nursing care including wound care, medication management, and patient education.
- Collaborate with patients, families, and caregivers to ensure holistic care delivery.
- Communicate effectively with interdisciplinary team members to coordinate patient care.
- Maintain accurate documentation of patient encounters and care provided.
- Active RN license in the state of Louisiana.
- Minimum of 6 months experience in home health nursing.
- Familiarity with HomeCare/HomeBase EMR system.
- Current BLS certification.
- Availability to work Monday through Friday day shifts.
- Ability to meet productivity expectations of 25-30 units per week.
- Valid driver's license and willingness to travel between patient homes, with mileage reimbursements provided.
