Job Description
Description: This position provides custodial and personal care services to clients that contract with Hibernian Home Care for services under the direction of the Staffing Supervisor in compliance with the Plan of Care.

Primary Duties:

Assist with Toileting, bathing, grooming, transfers, dressing, ambulatory, feeding and light housekeeping, Med reminder, cognitive activities.

Abides by the agency's infection control policies, including proper hand washing techniques consistent with CDC Guidelines.

Abides by all policies and procedures outlined in the agency's Policy and Procedure Manual, Caregiver Handbook, and Caregiver Training Manual.

Hours: Dependent on assignment; 12- hour Shifts available based on availability. Hibernian Home Care does not guarantee hours.

Locations:

Oak forest, Chicago, Hinsdale, Hickory Hills, Worth, La Grange, Oak Lawn, Oak Brook, Orland Park, Palos, Western Springs, Westmont, Hyde Park, Alsip, Evergreen Park. Will county, Cook County, and DuPage County.

Reports to: Home Care Supervisor
