    Kings Staffing is a career pathway for those interested in the Human Services industry.

    We are seeking an Enrollment Case Manager to enroll residents in Home and Community Based services.

    Responsibilities:

    • Travel throughout assigned areas to visit cases.
    • Conduct intake visits for waiver applicants and facilitate eligibility determination.
    • Present waiver programs, advise applicants of rights and responsibilities, and assist with application submission.
    • Investigate Medicaid eligibility and select Supports Coordinators that meet applicant needs.
    • Consult with Enrollment Nurses and develop Individual Service Plans.
    • Foster relationships with community organizations, advocates, and stakeholders to improve the application process.
    • Maintain relationships with parties serving mutual consumers, including physicians, County Assistance Offices, providers, nursing homes, and rehabilitation facilities.
    • Report on schedule availability and perform other duties as assigned by management.

    The ideal candidate will have a Bachelor's or Associate's degree in human services, relevant experience, excellent organizational, written, and verbal communication skills, and cultural sensitivity.

    Preferred qualifications include three years of human services or healthcare experience, staff management, knowledge of Pennsylvania Home and Community-Based waiver services (HCBS), and bilingual capabilities in American Sign Language.
