- Participating in mandatory rehospitalization-related events and seminars within the agency and communities
- Analyzing data, recognizing trends in hospital admissions, and delivering education to patients and staff to prevent hospitalizations
- Aiding in the development and execution of protocols and guidelines for Concierge Best Practices Programs
- Supervising virtual care and telehealth systems through assessments, education, and data monitoring
- Must hold an Unincumbered Florida Licensed Practical Nurse certification
- Minimum of two (2) years experience as a home health clinician
- Robust clinical background
- Experience in triage/preventing patient hospitalizations
- Proficiency in Microsoft Office
