Today, 60 million Americans living in rural communities are facing a crisis of access to care. In the U.S. healthcare system, rural Americans experience significantly poorer clinical outcomes. This trend is rapidly accelerating as rural hospitals close and physician shortages increase, exacerbating health disparities.

    In fact, Americans living in rural communities suffer a mortality rate 23 percent higher than those in urban communities, in part because of the lack of access to quality care.

    So, we're rearchitecting the way it's delivered - leveraging breakthroughs in diagnostics, remote monitoring, and a variety of other domains, Homeward is creating a new healthcare delivery model that is purpose-built for rural America and directly addresses the issues that have historically limited access and quality.


    Most importantly, we're aligning everyone's incentives (ours, partner health plans', and members') towards getting and keeping people healthy by taking full responsibility and financial risk for the total cost of each member's healthcare and outcomes.


    Homeward is co-founded by a leadership team that defined and delivered Livongo's products, and backed most recently by a $50 million series B co-led by Arch Ventures and Human Capital, with participation from General Catalyst for a total of $70 million in funding.

    With this leadership team and funding, Homeward is committed to bringing high-quality healthcare to rural communities in need.

    The Opportunity


    Join us in tackling healthcare for rural America We're seeking a VP, Clinical Strategy who is passionate about having a positive societal impact and using technology for good - solving issues of health inequity for communities that have been perennially underserved.


    You will be responsible for driving the evolution of our care delivery model, with the goals of optimizing member experience & retention, clinical quality, health, and cost outcomes.

    In addition to collaborating with corporate enablement functions to ensure success, you will partner closely with in-market teams to ensure our members and clients are reaping the benefits of this work.

    You will shape the company's direction as a leader, ensuring our solutions are clinically effective and that our clinical practices are aligned with the company's mission and values.

    What You'll Do


    • Lead care model vision and strategy and work closely with product, engineering, data science, regulatory, clinical operations and other teams to articulate and build new clinical delivery models
    • Employ a data-driven approach to strategy development, grounding clinical objectives and strategy in actionable insights and quantitative goals
    • Assist in building clinical workflows and utilization management tools to support high-quality care that is effective, engaging, and efficient for patients and providers
    • Create stakeholder buy-in for Homeward's care model vision by working closely with marketing, product management, and design teams
    • Oversee the clinical strategy team and provide guidance, leadership, and development opportunities to ensure high performance and career growth
    • Ensure that Homeward Health's clinical practices align with the company's mission and values and comply with all regulatory, ethical, and legal requirements related to clinical activities and trials
    What You Bring


    • An advanced medical degree (MD, but residency training and clinical practice experience preferred)
    • 7+ years of experience in a clinical strategy leadership role, preferably in provider organizations for new care models for value-based arrangements
    • A strong metrics-driven approach, with the ability to assess performance, pilot test solutions, track improvement, and scale high-impact solutions
    • Experience with primary care, including a deep understanding of the clinical needs of rural patients
    • Excellent communication and interpersonal skills, with the ability to effectively communicate with both clinical and non-clinical audiences
