How You Can Make an Impact

The Discharge Planner plays a vital role in coordinating and executing discharge planning services, which involve identifying local resources, setting up appointments with programs or providers, and offering psychoeducation sessions to individuals before their release into the community.

Key Responsibilities

• Provide discharge planning services from intake, covering MAT, medical, and behavioral health programs according to standards
• Conduct psychoeducational sessions for individuals preparing for release
• Cultivate relationships with community agencies offering mental health, substance use disorder, and medical services
• Assist with financial aid applications and coordinate benefits resumption to ensure medical needs post-release
• Maintain relationships with facility staff, document services, and comply with security and confidentiality protocols

Qualifications & Requirements

Education

• Bachelor's degree in social work, psychology, or related field; nursing license (LPN or RN) preferred
• High school diploma accepted with care coordination and correctional experience

Experience

• Proficient in creating transition plans involving community resources for individuals with medical, substance use, or mental health needs; correctional background preferred

Licenses/Certifications

• CPR certification and any other required certifications
