    ResponsibilitiesThe Home Health Aide (Certified Nursing Assistant, CNA) is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional well-being, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures.

    Follows current written aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs) to include mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting.

    As assigned, performs incidental household services essential to the patient's health care at home that are necessary to prevent or postpone institutionalization.

    Completes a clinical note for each visit within required timeframes, which must be incorporated into the patient's record.

    Provides care according to the aide plan of care/assignment sheet and in accordance to State Practice Act or regulations.

    Education and ExperienceLicense Requirements

    Successful completion of a competency evaluation.
    Current CPR certification required.
    Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

    Additional State Specific Requirements


    TX:

    Must have a minimum of one-year full-time experience in direct client care in hospital or nursing facility, OR one year full- time experience within last 5 years in direct client care in agency setting, OR documented completion of training and competency evaluation program with home health experience within the last 24 months, OR documented completion of competency evaluation program with home health experience within the last 24 months, OR nursing student with documentation from director of programs or the dean of a school of nursing that states that the individual is a nursing student who has demonstrated competency in basic nursing skills aligned with the school's curriculum, OR be on Texas Department of Human Services (DHS's) Nurse Aide Registry with no findings of client abuse or neglect or misappropriation of client property.

    Must NOT be listed on the Employee Misconduct Registry, been revoked on the Nurse Aide Registry, or had a criminal conviction as listed in Health and Safety Code Chapter 250.

    Skill Requirements

    Must be mature and able to deal effectively with the demands of a home health aide position. Able to read, write, and carry out directions promptly and accurately.
