    RESPONSIBILITIES

    • Assist clients through the program by operating as a client advocate and systems navigator.
    • Provide comprehensive case management that includes screening and assessing clients; creating, implementing, and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing, and other community resources.
    • Continually monitor and evaluate each client's progression through their case management plan and develop corrective action revisions to the plan as needed.
    • Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with established guidelines.
    • Promotes clear communication amongst interdisciplinary care team members by ensuring awareness regarding patient care plans.
    • Participates as a successful team associate supporting data collection, health outcomes reporting, clinical audits, and pragmatic evaluation.
    • Other duties as assigned.

    QUALIFICATIONS

    • Bachelor's degree in human service or comparable combination of education/work-related experience in a social service setting; working knowledge of case management and planning techniques
    • Reliable vehicle and active Driver License/Car Insurance required
    • A team player that can follow a system and protocol to achieve a common goal
    • Highly organized and well developed oral and written communication skills
    • Confidence to communicate and outreach to other community health care organizations and personnel
    • Demonstrates sound judgment, decision making and problem solving skills
