Shift*:
12 Hour shifts, self-scheduling

*About the Unit

• Reporting to the Practice Coordinator, this position is responsible for assessing, planning, implementing and evaluating the nursing care of each assigned patient; supervising, coordinating and guiding the activities with nursing staff members; and implementing the philosophy, goals and standards of care of the unit.
• Ability to communicate effectively with patients, family, and health care personnel is required.
• Delivers individualized quality patient care through a coordinated team approach with physicians, staff, other departments and the patient's family and/or significant others.
• Effectively engages patients, families and/or significant others regarding the plan of care, short and long term goals, progression toward goal achievement, including discharge planning.
