- Complete in-home visits following program guidelines.
- Evaluate clients for mental and physical health barriers by performing a variety of assessments.
- Ensure nursing assessments are completed according to program guidelines.
- Documentation will occur in real time at client visits, with full completion in 48 hours.
- Audit documentation, revise, and complete by deadline given.
- Maintain accurate documentation per program policies.
- Provide education on maternal child health, safe sleep, substance use cessation, child safety, family planning, mental health, and other resources, as applicable.
- Conduct developmental screenings per program timeframe.
- Complete referrals to other community agencies based on individual client needs.
- Participate in a minimum of 3 outreach events or coalition meetings per quarter, at least one of which must be an outreach event.

This position works to meet the mission of the Sedgwick County Health Department by promoting the wellness of Sedgwick County residents through the Healthy Babies Program's focus on reduction of infant mortality, maternal mortality, low birth weight, and prematurity among high-risk birthing people.
Employee may deploy to the field as a responder or in a leadership position during public health emergencies (Activation Level 3).
Home and community visitsMinimum Qualifications:
Associate's Degree in Nursing. One year experience in nursing. Current Registered Nurse (RN) license in the state of Kansas required. Attends required trainings to maintain licensure.
Per Sedgwick County Policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance.
Must complete Federal Emergency Management Administration (FEMA) IS 100, 200, and 700 courses within 30 days of employment.Preferred Qualifications:
Bachelor's Degree in Nursing (BSN). One year experience post-BSN degree in nursing. Home visiting experience.
A minimum of one year working in either clinic or hospital settings:
labor and delivery, postpartum, OB-GYN, pediatrics, and/or public health. Knowledge in early childhood development and prenatal education. Current Registered Nurse (RN) license in the state of Kansas required. Attends required trainings to maintain licensure and/or certification. CPR certification within the first 90 days of employment. Bilingual English/Spanish skills.
