- Provide direct patient care as instructed by the Medical Director
- Perform duties like histories, physicals, ordering tests and medications
- Conduct primary care visits, develop treatment plans, and coordinate care with the primary care behavioral health team
- Ensure accurate and timely medical records documentation
- Contribute to enhancing clinic procedures and policies
- Utilize the Electronic Health Record system effectively to meet documentation requirements
- Current Family Nurse Practitioner (FNP) license with prescribing rights
- Minimum two years of independent practice in primary care
- Approval by the clinic's Professional Liability Insurance provider
- Experience in trauma-informed, client-centered care and harm reduction practices
- Ability to provide culturally competent care to homeless, indigent, transgender, gender diverse, and LGBTQ+ patients
- Familiarity with state healthcare laws and licensing regulations
- Effective communication and strong computer skills
- Current BLS certification and bloodborne pathogen training
- Experience in caring for homeless or low-income populations
- Fluency in English and Spanish

Compensation Details:
