Description
This position will be working four 10 hour shifts a week to support the Transitional and Extended Care facilities.
This position is responsible for providing safe transportation to residents in a timely, competent manner, and for communicating important information relevant to the overall care plan to the multidisciplinary team.
The position is critical to setting the tone and desired experience that Providence Extended Care and Providence Transitional Care Center expects for each participant on outings and other appointments.
