Job summary
We are seeking a compassionate Full-Time Social Care Navigator to join our team in the Social Care Network Department. In this role, you will be responsible for conducting HRSN screenings and eligibility assessments. This is a grant funded position. Location is remote however applicant must be able to report to West Babylon NY for trainings and or meetings. We offer comprehensive outpatient mental health and addiction services.BENEFITS INCLUDE:
