Job summary
The Care Coordinator is a member of the healthcare team and is responsible for coordinating, communicating,and facilitating the clinical progression of the patient's treatment and discharge plan.
A day in
the life of a Care Coordinator includes assessing patients by screening for potential discharge needs,
regardless of race, age, sex, religion diagnosis and ability to pay., Facilitates communication
and coordination between members of the healthcare team and involves patient/family in decision making process
Maintains current information on community resources referring patients to those appropriate for care consults with other agencies committees to identify resources supporting patients families Works collaboratively with multidisciplinary post acute teams securing timely transitions Ensures that medical record reflects education provided coordination referrals made authorizations obtained Participates actively in appropriate committees workgroups meetings Identifies refers quality issues review Quality Management Program Performs reassessments evaluates progress against goals revises plan as needed Provides patients families with resources options educates regarding risks benefits discharge options available health care benefits Utilizes social determinants health screening tools during each intake assessment Collaborates supports crisis intervention counseling support referrals abuse neglect reporting adoption planning guardianship psychosocial assessments observation management capacity hospital throughput Refers as required needed Acute rehabilitation facilities Sub Acute rehabilitation facilities Long Term Care facilities etc.
