Job Summary
This position is responsible for all functions of case management including advocacy coordination assessment linking monitoring of services for an assigned caseload of individuals with a primary diagnosis of developmental disability or mental illness.- Gathers and Reviews information such as medical psychological social history contributing to the individual's situation.
- Initiates coordinates professional assessments according to presenting problem relevant standards.
