Job summary
We are seeking licensed clinicians to join our team at Open Sky Community Services. Our Adult Community Clinical Services department delivers evidence-based interventions in a clinically focused model. We help individuals with mental illness or dual diagnosis transition to independent living while maximizing their natural supports.Responsibilities
- Complete screening and enrollment of persons served in compliance with DMH regulations.
- Develop and implement Comprehensive Assessments and Individualized Treatment Plans using Person-Centered Planning.
- Build relationships with individuals served and develop shared understanding of Person's goals.
