We deliver exceptional personalized care that improves health lowers costs and empowers people to live their best lives With deep roots in clinical assessment and care management services our national network of clinicians breaks through traditional barriers to care by meeting those members wherever they call home Our providers conduct adult geriatric assessments including medical history diagnosis treatment health education physician referrals case management referrals follow up and clear documentation according to Matrix guidelines and protocols They administer order point of care lab-based testing as needed Work with physicians case managers social workers family members key caregivers ancillary medical personnel Collaborate with Primary Care Physician PCP on patient education and follow up as necessary Provide services at the member's place of residence which can be in their homes or Skilled Nursing FacilitiesResponsibilities
