    Overview

    Functions as a pharmacist as defined by the Iowa Board of Pharmacy Examiners. Responsible for providing all aspects of pharmaceutical care for patients served within our health system. Performs assignments and projects as directed by the Pharmacy Management Team. Will perform functions across all UnityPoint Health - Des Moines retail locations.

    Responsibilities

    Operational Functions

    o Oversees technical and support staff in the absence of administrative personnel


    o Reviews the work of other pharmacy personnel to ensure the appropriate dispensing of medications per the prescriber's order in accordance with laws and regulations.

    o Coordinates the scheduling of pharmacists and pharmacy technicians

    o Supervises the work activities of pharmacy technicians

    o Creates and maintains patient profiles within the pharmacy system

    o Ensures appropriate and adequate medication inventory of controlled and non-controlled drug products by restocking, rotating stock, removing expired medications, packaging medications, and ordering supplies

    o Ensures proper medication storage

    o Assists in finding cost-savings opportunities for patients, including coupon cards

    o Inspects pharmaceutical supplies, storage, and utilization

    o Maintains a neat, clean, orderly, and secure department and work area

    o Prioritizes tasks to decrease patient wait times for prescriptions and ensure efficiency within the department


    • Clinical Functions
    o Practices pharmaceutical care activities. Reviews medication orders to assess the appropriateness of drug selection for the patient. Based on patient specific parameters (e.g. renal and hepatic function, weight, co-morbidities, patient-specific medication compliance issues, etc.), evaluates medication regimens for appropriate dosing, schedule, route, contraindications, and potential drug interactions. Contacts the prescriber as necessary for clarification or to recommend appropriate therapy or dosage changes.

    o In accordance with established policies, procedures and protocols, specific activities may include, but are not limited to:

    o Dosing of medications using the principles of pharmacokinetics

    o Provide drug and disease education

    o Evaluation of complex questions regarding medication regimens and adverse drug reactions

    o Assessment of medication treatment guidelines, protocols, medication formularies and dosing guidelines, identifying areas for improvement in these areas

    o Verifies the accuracy of order entry and product dispensing

    o Ensures that written, electronic, and faxed prescriptions comply with state and federal requirements

    o Documents pharmacist interventions in the electronic health record

    o Provides drug information to patients, community members, and the healthcare team

    o Counsels patients on new prescriptions and provides information about the drug product, including indications for use, side effects, administration instructions, and duration of therapy

    o Performs Medication Therapy Management (MTM) services

    o Reviews medication profiles with each prescription fill

    o Assists patients with non-prescription products

    o Participates in vaccine screening and administration

    o Supports the expansion of patient care services

    o Supports the ongoing development of pharmacy staff to increase knowledge and skills

    o Demonstrates a commitment to the improvement of patient care, customer satisfaction, and team member performance

    o Participates in the experiential education of pharmacy students and residents

    o If applicable, designs and participates in Collaborative Practice Agreements (CPAs)

    o Ensures the appropriate preparation, dispersal, and security of medications in accordance with facility, state, and national laws and regulations

    o Assures compliance with HIPAA, DNV, OSHA and other state and federal regulations guiding healthcare

    o Adheres to policies and procedures of UPH-DM

    o Prioritizes the importance of preventing errors, accidents, and infections within the workplace and reports potential or actual concerns immediately

    Qualifications


    Education:

    • Doctor of Pharmacy (Pharm.
    D.) or B.S. Pharmacy with equivalent clinical experience

    • Completion of an ASHP PGY-1 pharmacy residency in community practice or ambulatory care desirable

    Experience:

    • Must be able to proficiently read, write and speak English
    • Proficiency in navigating the electronic medical record
    • One or more years of community pharmacy experience preferred
    • One or more years of hospital pharmacy experience desirable

    License(s)/Certification(s):

    • Must maintain a current license to practice pharmacy in Iowa and be in
    • Completion of the APhA Immunization Training Certificate Program desirable
    • Board Certification desirable
