Role Description
This is a

contract role for a Care Manager, in Oklahoma City.

As a Care Manager, you will be responsible for providing case management services to individuals and families. This includes assessing client needs, developing care plans, coordinating services, and monitoring progress. You will also provide training and education to clients and their families on care management best practices.
Qualifications
Case Management and Elder Care skills
Experience in social services or related field
Knowledge of medical terminology and healthcare systems
Excellent communication and interpersonal skills
Ability to work independently and remotely
Strong organizational and problem-solving skills
Experience in training and education
Bachelor's degree in a related field (e.g. Social Work, Nursing, Psychology) is preferred
