    Unit Overview

    Staffing Model
    • Total CM Staff Needed: 12–13 RN Case Managers (full coverage)
    • Schedule: 8-hour shifts, 5 days per week
    • No Night Shift

    Patient Ratios

    • Case Managers:
      • 1:25 (floors)
      • 1:20 (UC units)
    • Social Work:
      • 1:30 (ICU)
      • 1:40 (floors)

    Equipment & Systems

    • iPad, computer, cell phone
    • EMR: Meditech (chart review)
    • Documentation: Midas

    Responsibilities

    • Patient assessments and evaluations
    • Discharge planning with patients and families
    • Care coordination
    • Participation/leadership in multidisciplinary rounds (MDRs)

    Required Qualifications

    Must Have

    • Hospital and/or acute case management experience
    • Active RN license

    Preferred

    • Experience with Midas, InterQual, Meditech
    • Acute hospital case management background

    Schedule & Expectations

    • 8-hour day shifts (8:00 AM – 4:30 PM)
    • Every other weekend required
    • Holidays worked in rotation with hospital staff
    • Scheduled according to facility policy
