Hospice Nurse Case Manager

We are in search of a caring Hospice Nurse to provide care and assistance to patients nearing the end of their lives.

A Hospice Case Manager nurse is a registered nurse (RN) responsible for overseeing the care of hospice patients. They also educate families and caregivers on identifying potential symptoms and providing competent and safe care for the patient.

Hospice RN Case Managers also offer practical and emotional support to patients, their families, and caregivers.


Hospice Nurse Responsibilities:
Supervising the care of patients approaching the end of their lives.

This includes, but is not limited to, scheduling patient visits based on their needs, collaborating with physicians, managing orders, and overseeing medication management.

Providing supervision for LPNs/LVNs/Hospice Aides according to established protocols.

Working collaboratively with a team of medical professionals to ensure that patients receive high-quality palliative care, encompassing physical, emotional, and spiritual support.

Qualifications:

Preferred:
Associate's degree


Required:
Registered Nurse license

At least one year of experience
Prior experience in hospice care
Availability for on-call duties
