- Provide primary care and services to Longevity Health Services members
- Implement Longevity Health Model of Care activities
- Administer Health Plan clinical operations functions
- Act as a trusted advisor and advocate for members
- Collaborate effectively with members' PCPs, families, and care teams
- Promote Longevity Health within the community
- Work autonomously and be accountable for practice
- Establish meaningful relationships in the nursing home environment
- Focus on member-centric and high-quality care
- Assist with other assigned duties
- State licensure as a Nurse Practitioner or Physician's Assistant
- National Board certification required
- DEA License necessary
- Active NPI and Medicare provider numbers
- Valid driver's license and auto insurance
- Education: Master's Degree in Nursing or equivalent
- Experience in healthcare and working with the geriatric population
- Proficiency in technology and strong communication skills
- Ability to work independently and in a team
- Strong clinical skills and critical thinking abilities

LiveWell Choice, a division of Longevity Health, is currently expanding and seeking to recruit an Advanced Practice Provider (APP) - Nurse Practitioner or Physician Assistant - Floater in Greenville, SC.About the Position: The Advanced Practice Provider (APP) - Market Floater is a licensed and board-certified healthcare professional assigned to Longevity Health members to implement the Institutional Special Needs Plan Model of Care. These professionals are not permanently based at a single facility but rather move within a state market to address staffing needs. Travel schedules vary by market and are mutually agreed upon by the MDCS and the new hire. APPs needing supervision are hired based on available physicians and oversight processes in each state where Longevity Health operates.
