Provide compassionate, appropriate and effective clinical care for patients of all ages. - 100% outpatient care if not providing OB services.
- Maintain good quality medical records.
- Work within a team collaborative model at a Patient Centered Medical Home.
- Advocate for quality care on behalf of patients and the organization.
- Provide care and treatment.
- Diagnose, treat, and monitor diseases, disorders, and conditions.
- Prescribe appropriate medications to treat patients in accordance with standardized procedures and company policy.
- Perform pap smears, pelvic and breast exams.
- Conduct mood and emotional well-being assessments during post-partum appointment.
- Collaborate and consult with other medical and nonmedical personnel.
- Effectively monitor patients' progress throughout treatment plan and care, and provide continuous improvement on patient outcomes.
- Educate patients on the importance of treatment plans and communicate side effects and risks of medications prescribed.
- Educate patients on disease prevention and health.
- Demonstrate effective and detailed written documentation skills.
- Conduct routine check-up appointments and maintain follow-up throughout
- Demonstrate excellent customer service and communication with patients.
- Able to multitask and manage assigned projects and tasks as directed.
- All other duties and responsibilities as assigned.
- MD/DO degree.
- Successful completion of family medicine residency.
- Licensed in the state of New York and/or Pennsylvania or willingness to obtain licensure.
- Board Certified or Board Eligible.
- DEA Certification required.
- Ability to demonstrate excellent independent judgment, problem-solving, analytical, and collaborative skills in order to organize and prioritize multiple tasks, providing patient-centered leadership and direction.
- Must have the emotional intelligence that may include, but not limited to, patience, sensitivity, and the compassion to work with patients that may be stressed, anxious, and difficult.
- Ability to work as a team.
- Must have strong computer skills.
- Must demonstrate detailed, effective writing/documentation and verbal skills.
