Job summary

We are seeking a Social Work Care Coordinator to assess key needs and assist families in setting appropriate goals and tracking progress. The ideal candidate will navigate the healthcare system, supervise care coordination, plan education, and promote patient advocacy.

The successful candidate will have experience as a clinical or medical social worker with knowledge of Medicaid standards. A Master's degree in Social work is required along with active BLS certification.
