    Summary:
    The Pharmacist is responsible for supervising and coordinating all activities of personnel in the hospital pharmacy.

    This individual also is responsible for compounding and dispensing medications by means of standard physical and chemical procedures to fill written medication and prescription requests issued by all qualified prescribers, records maintenance, Pharmacy Technician supervision, and quality assurance.

    Physical requirements may include long periods of standing, high mental pressure, responsibility and tension. The work environment is a hospital setting and may include exposure to potentially hazardous and toxic substances. Performs any duties and/or responsibilities assigned by Manager or Supervisor for which qualified.


    Responsibilities:

    • Ensures pharmacy activities meet all hospital policies, standard practices of the profession and state and federal regulations.
    • Initiates, develops and implements rules and regulations pertaining to administrative and professional policies of the department with the approval of the administration.
    • Assists with the coordinating of ordering, receiving and utilizing of supplies and equipment for the pharmacy.
    • Supervises the management of inventory stock and ensures accurate dating/expiration system is in place.
    • Ensures that an accurate charging system is operational for pharmacy items.
    • Participates in the Quality Assurance program.
    • With the Director of Pharmacy, ensures The Hospital's compliance with JCAHO requirements and State Commission of Pharmacy standards.
    • Prepares timely reports on the status of pharmacy projects, needs or problems as requested.
    • Attends all administrative/supervisory meetings as required.
    • Interprets orders, prescriptions and drug requisitions and dispenses pharmaceuticals and medications as requested by prescribers by means of appropriate procedures. Maintains all records pertaining to this activity.
    • Provides patient care areas with up to date medication information, which will be used to assist patients going home to better understand their therapy.
    • Reviews all prescriptions, drug requisitions, and direct physician orders to determine completeness, and assure that overdose, toxic agent incompatibilities, allergies or other interactions are not involved before administering the initial dose. Consults with appropriate party to clarify or correct information if necessary.
    • Assists with a computerized medication record for all patients in nursing areas assigned to Unit Dose systems of medication administration and reviews this profile for any potential interactions, interferences, or problems concerning treatment.
    • Oversees monthly inspections of patient areas and files necessary reports and records in accordance with procedure.
    • Supervises activities of Pharmacy Technicians. Ensures that other employees are not assigned duties that, by law or regulation, must be performed by a licensed Pharmacist.
    • Prioritizes drug orders and requisitions to best utilize system of distribution and personnel.
    • Monitors the handling, storage and distribution of all medications and chemicals to ensure safety, efficacy and proper utilization. Identifies and corrects any drug defects, outdated materials, mislabeling and misbranded medications or drug hazards.
    • Maintains confidentiality of all departmental information, security arrangements and precautions, and patient related information. Ensures method of preventing theft or diversion of medications.
    • Reviews narcotic dispensing records, administration records, and receiving and recording records/materials to ensure proper security and adherence to proper procedures as required to prevent violations of laws and policies.
    • Maintains hospital formularies and other sources of information on preparations, standard compendia on pharmaceuticals, reference texts and journals.
    • Participates in all patient care evaluations, drug audits and surveillance programs that require pharmacist input.
    • Consults with medical staff regarding information on medications, drug warnings, incompatibilities and contraindications on a timely and accurate basis.
    • Serves on committees as requested.
    • Maintains an awareness of new pharmaceutical advances and products and informs the appropriate hospital staff.
    • Interviews, hires, and trains/orients employees.
    • Allocates responsibilities to ensure maximum productivity and teamwork.
    • Provides access to inservices and continuing education for department personnel. Promotes professional. Promotes professional growth and development.
