Home Health Certified Occupational Therapy Assistant (COTA) –PRN | Dallas, TX
PIVOT Rehabilitation is hiring a Licensed Home Health Certified Occupational Therapy Assistant (COTA) for a
PRN home health occupational therapy assistant job providing in-home occupational therapy services to adult and senior patients in Dallas, TX and surrounding DFW areas.
This home health COTA job is ideal for clinicians seeking flexible occupational therapy assistant jobs in Dallas, autonomy in the field, and meaningful one-on-one patient care in the home setting.
Provide skilled home health occupational therapy interventions under the supervision of a licensed Occupational Therapist (OT)
Implement and carry out individualized Plans of Care (POC) developed by the supervising OT
Provide treatment for orthopedic, neurological, cognitive, and post-surgical conditions
Address ADLs and IADLs, including bathing, dressing, grooming, toileting, feeding, and home management tasks
Facilitate improvements in upper extremity strength, coordination, ROM, balance, and functional mobility
Implement fall prevention strategies and home safety interventions
Train patients and caregivers on adaptive equipment, assistive devices, and durable medical equipment (DME)
Recommend environmental modifications to improve safety and independence
Complete accurate and timely documentation in the EMR
Communicate effectively with the supervising OT, physicians, and interdisciplinary team members
Follow all home health regulations, infection control standards, and safety guidelines
Participate in QAPI and agency performance improvement initiatives

Qualifications
Qualifications
Associate Degree from an accredited Occupational Therapy Assistant program
Active Texas Certified Occupational Therapy Assistant (COTA) license
Home health occupational therapy experience preferred (1+ year preferred)
Knowledge of Medicare home health documentation standards preferred
Current CPR certification
Strong communication, clinical judgment, and time-management skills
Valid driver's license, reliable transportation, and current auto insurance
Ability to travel throughout Dallas and the surrounding DFW areas
