    You'll provide home-based skilled speech therapy care for patients as directed by the attending physician. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.

    Patient Outcomes:

    Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.


    Minimize Patient Risk:
    Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision.

    Family Educator/Advocate:
    Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.

    Policies:
    Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
    Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.

    Operations:
    Attend in-service training and mandatory agency meetings.
