    • FULL TIME - Day Shift - 40 hrs/week
    • Sign on bonus - $15,000 for RNs with 1+ years of experience
    This is NOT an office position. This is a field clinician who is seeing

    patients in the home.

    WORK EVERY 3RD WEEKEND

    2 HOLIDAYS PER YEAR ON A ROTATION

    WHEN NOT SCHEDULED WEEKEND, WORK MONDAY – FRIDAY DAY SHIFT

    FLEXIBILITY WITH SCHEDULING TO ACCOMMODATE APPTS.

    CASE MANAGEMENT LOAD OF 15-20 PATIENTS

    JOB DESCRIPTION:


    The Home Care Case Manager is a registered nurse who applies nursing knowledge and clinical skills using the nursing process to ensure quality and safe delivery of home care services.

    The Home Care Case Manager possesses strong clinical skills and a knowledge base in a specialized area and utilizes leadership skills in coordinating and/or providing home care services for a group of patients within that specialty area.

    These services include education of patients/families and staff, collaboration with the physician in the plan of care, total coordination of patient care and the quality monitoring of appropriateness of patient care.

    The group of patients can include home care patients of all ages. The Home Care Case Manager facilitates achievement of outcomes.


    The Home Care Case Manager is a registered nurse with a minimum of 2 years recent acute care or home/hospice care experience.


    EDUCATION:
    Graduate from accredited School of Nursing

    Current RN license in State of Ohio

    Must possess BSN or obtain within 5 years of employment


    EXPERIENCE:
    Orientation program is provided

    Computer literacy

    BLS certified by American Heart Association

    WORKING CONDITIONS
    Periods of high stress and fluctuating workloads may occur; General office environment - works generally at a desk in a well-lighted, airconditioned office area, with moderate noise levels; May be exposed to limited hazardous substances or body fluids; Potential exposure to infectious organisms; May have periods of constant interruptions; Prolonged periods of working alone may occur
