Founded in 2003, Hometown Health Center (HHC) is a healthcare leader offering over 32,000 medical, dental, and behavioral health visits annually to 6,200 patients. As a Federally Qualified Health Center, we take pride in providing essential services to those in need of medical, behavioral health, and dental care. HHC is expanding with a new cutting-edge facility in Palmyra.
The Addiction Medicine Provider will deliver onsite medical services. This role involves implementing treatment plans in collaboration with the supervising physician and team, conducting patient assessments, and monitoring patients at all stages of addiction treatment. By positively impacting the lives of individuals with substance use disorders, this role plays a crucial part in our community.
