- Build effective working relationships with members, providing support and connection to medically necessary services. This includes meeting with members in the community, in office, and providing transportation to medically necessary care.
- Educate and assist members in navigating the behavioral health and medical health care systems.
- Build connections between members and their multidisciplinary care providers. Identify and collaborate with member and providers on member's physical, emotional, and cultural needs and help them access appropriate resources to meet these needs.
- Act as a part of the member's treatment team and have a good understanding of the health resources available in member communities in order to refer members to the best resources available to them.
- Assist members in overcoming barriers in accessing health care, including, but not limited to, insurance or lack thereof, poor health literacy, transportation, childcare.
- Collaborate with member and their family, guardian, HCDM, or others to implement effective service planning, explaining available options and resources, including advantages and disadvantages of each option.
- Maintain member's comprehensive clinical record, including documentation of activities performed as part of the service delivery process. This includes both billable and nonbillable activities.
- Provide continuous evaluation of the level of care determination and make recommendations for changes in level of care to supervisor.
- Adhere to agency policies and procedures. Comply with agency's standard care of care for service delivery. Assist with Quality Management, Quality Assurance, and Utilization Review.
- Deliver at least 40 hours of work per week, with a goal of 30 hours of direct service. Productivity standards are established annually, at a minimum.
- Perform additional related duties and participate in agency fund-raising activities, as requested.

- Minimum of High School Diploma or GED equivalent is required.
- Bachelor's degree in Psychology, Social Work, Family Studies, or other behavioral health field preferred.
- Medical Assistant Certification or Certified Nursing Assistant, preferred.
- Minimum of one year experience in provision of direct behavioral health services.
- Experience in integrated healthcare setting preferred.
- Experience conducting outreach and engagement with members determined to have a serious mental illness preferred.
- Experience in coordination of care with behavioral health and medical providers.
- Must be 21 years of age or older at time of employment.
- The typical work environment will be working indoors or community-based work, working out in the community.
- Employees must be able to lift up to 15 pounds at times.
- Occasional travel to training and out-of-state locations may occur as-needed.

What you'll do:
The purpose of the Health Navigator position is to coordinate with the SMI treatment team to gather and share member health information under an integrated care model to improve member healthcare outcomes. The Health Navigator is part of a member's SMI treatment team and serves as a client advocate. The position includes working in the SMI clinic and community, navigating clients who may be facing homelessness, justice involvement, serious medical or mental health diagnoses, and/or challenges in everyday functioning. The Health Navigator is responsible for coordinating care with, and on behalf of, a member to address barriers to care and wellness.
