Matrix providers conduct comprehensive health assessments during visits, gaining valuable insights into members' well-being beyond routine office visits, potentially closing multiple care gaps in one visit.

Nurse Practitioner - Home Risk Assessments
Type: PRN-Flexible-Nurse Practitioner
Location: Home, SNF, Telehealth in regions like Whitney, Texas
Hours: Flexible scheduling including days, evenings, nights, and weekends
PAY: $95.00 or $100.00 per completed Assessment, 25 hours or more/month
Benefits: Non-benefited PRN roles with limited voluntary benefits
Mileage reimbursement and incentives for completed assessments
