    About Community Healthcare Network (CHN):

CHN is a New York City-based federally qualified health center providing access to quality, culturally competent, and comprehensive community-based primary care, mental health services, and social services. As a network of twelve health centers across four boroughs, multiple medical mobile units, and two school-based health centers, CHN reaches over 85,000 individuals each year.

CHN has received funding from Health Resources and Services Administration ("HRSA") to establish a new accredited family medicine residency program. CHN intends to develop this program in Jamaica, Queens, where it currently operates two health centers. This new residency program will enroll 4-6 residents in its first academic year and contribute to increasing community-based provider education and workforce development opportunities within medically underserved communities. CHN expects to apply for accreditation in the end of 2022 and enroll its first resident class in the fall of 2024.
