Your Role as a Home Health Physical Therapist

In this dynamic position, you will:

• Deliver home-based physical therapy to patients recovering from illness or injury.
• Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care plans and goals.
• Conduct assessments, document progress, and adjust care plans as necessary.
• Guide patients through exercises and hands-on therapy to enhance mobility and manage pain.
• Educate patients and their families on care plans, goals, and self-care techniques.

What We're Looking For

To thrive in this role, you should have:

• A degree from an accredited Physical Therapist Program and an active PT license in Michigan (New grads are welcome)
• A minimum of 1 year of clinical experience, preferably in home health within the last 5 years.
• Current CPR certification.
• Strong clinical judgment, interpersonal skills, dexterity, and compassion.
