    Our established, 50 year old company, is searching for a caring, experienced Clinical Supervisor for our Hospice Division.
    Job Description Summary
    The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor provides liaison services, coordinating patient transition to home health and/or hospice.

    Assists in the clinical orientation of all new employees, conducts field supervisory visits and competency assessments for annual clinical evaluations.

    Participates in the administrative on-call rotation.

    In the absence of the Administrator/Branch Manager during temporary absences and/or vacancies will assume temporary role of Administrator/Branch Manager in his or her absence.

    Essential Job Functions/Responsibilities

    Maintains a working knowledge of current Hospice Conditions of Participation, coverage guidelines and state regulatory requirements: admission criteria, eligibility and documentation requirements.

    Demonstrates leadership qualities, serves as role model in setting positive, attainable expectations, and goals for the clinical teams.
    Complies with accepted professional standards and principles.
    Clearly communicates performance expectations and hold staff accountable; fosters a sense of commitment and ownership among staff.
    Fosters positive work environment and culture that promotes unity and collaboration between home health and hospice.
    Reviews all intake referrals received by the agency intake department. Reviewing available pertinent information related to referral to determine home health or hospice needs. Assigns appropriate home health or hospice personnel to all valid referrals as needed.

    Reviews and evaluates each referral though a variety of means such as home visits, conference, record review and the services provided by clinicians; discusses and verifies impressions, instruct and guides clinician to promote more effective performance and delivery of quality home care and hospice services; and is available at all times during operating hours to assist clinicians as appropriate.

    Reviews all Hospice Assessments, clinical coordination notes, documentation, physician orders, plans of care and medication profiles for accuracy and compliance via electronic medical record documentation.

    Ensures that the medical records and all clinical documentation is maintained in compliance with federal, states regulations and company Policies and Procedures.

    Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care; serves as clinical resource to team and to referral sources.

    Ensures clinician compliance with documentation requirements, timely submission of documentation, corrections of documentation errors. Ensures clinician compliance related to physician orders.
    Oversees clinical operations for hospice; ensures that staffing and productivity are in-line with revenue and census.
    Prepares for and facilitates case conferences and bi-weekly IDT Hospice meetings to facilitate patient needs and coordination of care.
    Provides liaison services to facilities and to patients/families that have been referred to hospice services.

    Assists VPO, Administrator or Clinical Manager in ongoing focus and results related to revenue and census growth for Hospice programs.

    Promotes Hospice services in community; participates in selected community education/outreach activities.

    Participates in public relation and community activities that promote the organizations role as an effective member of the hospice care delivery system.

    Promotes customer service orientation to all hospice personnel.
    Collaborates with Clinical Manager, Home Office Team in employee recruitment, interviewing, hiring and termination processes for Hospice.
    Oversees Hospice employee orientation; delegates orientation activities as appropriate.
    Evaluates employee performance and provides ongoing feedback; initiates coaching/counseling process to address performance issues.
    Coordinates annual evaluations, field supervisory visits and competency assessments with other Clinical Supervisor(s), Intake Coordinators or other appropriate personnel.
    Addresses Hospice staff learning and education needs through annual educational plan.
    Participates in QAPI program
    Participates in sponsored in-service training
    Other duties as assigned by the Administrator or Clinical Manager.
    Position Qualifications
    Registered nurse.

    Previous experience in hospice with at least two years of management experience; demonstrated ability to work with an interdisciplinary team.

    Demonstrated ability to supervise and direct clinical and administrative personnel.
    Ability to market and deal tactfully with customers and the community.

    Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

    Has excellent observation, verbal and written communication skills.
    Knowledge of business management, governmental regulations.
    Complies and maintains current CPR certification.
