    Job DescriptionSalary: $24.00/hour

    Summary Description:

    Provide feedback and supervision to Direct Support Professionals, provide direct care to all residents focusing on daily living skills and provide case management to program residents.

    Duties and Responsibilities:

    I. Agency Administration

    1. Serves as the secondary person in charge, gives direction as needed, to regular and relief staff.
    2. Prioritize daily training, recreation and community inclusion activities.
    3. Monitor daily record sheets.
    4. Assure quality in program implementation, on a day-to-day basis.
    5. Meet weekly with the Program Manager to address issues, concerns, and program needs.
    6. Participate in regular supervision with the Program Manager.
    7. Monitor staff's ISP and progress note documentation.
    8. Participate in agency-wide projects on an as-needed basis.
    9. Assist Program Manager in scheduling shifts.
    10. Assists the Program Manager to ensure staff trainings are completed and documented.

    II. Financial Monitoring and Supervision

    1. Teach budgeting skills to individuals.
    2. Maintain contact with SII/SSA and insure regular ongoing reporting of all necessary information, or provide assistance to guardian/conservator in their reporting tasks.
    3. Ensure regular Medicaid/medical coverage.
    4. Maintain personal financial records (daily cash on hand records, bankbooks, etc.) in accordance with the individual's need for monitoring.

    III. Medical

    1. Maintain up-to-date DDS required medical and dental records, sending all reports to DDS and the individual's day program.
    2. Arrange and attend all medical appointments.
    3. Maintain optimal medical care for all residents.
    4. Respond quickly and appropriately to all medical emergencies.
    5. Obtain First Aid and CPR training within three (3) months of employment and update as needed.
    6. Obtain Medication Administration Certification within three (3) months of employment and update as needed.
    7. Follow MAP guidelines for all areas of Medication Administration.

    IV. Individual Service Plan (ISP)

    1. Write annual ISP according to accepted DDS standards and regulations.
    2. In consultation with other staff, interested family members and the DDS service coordinator, develop meaningful programs, which will serve as the chief component and measure of progress for the individual in their participation in the Residential program.
    3. Consult with supervisor and staff regarding goals and training modalities appropriate to individuals.
    4. Write and submit regular monthly or quarterly progress notes.
    5. Maintain all semi-annual records.
    6. Train all staff in implementation of individual training programs for each individual.
    7. Complete DDS Health Care Records
    8. Complete Assistive Technology Assessment and other required/necessary assessments.

    Family and Advocates Liaison

    1. Maintain contact with family members to report progress and concerns and provide an appropriate link to facilitate healthy family involvement.
    2. Maintain regular contact with day program to report significant issues, medical appointments and activities.

    VI. Religious/Cultural

    1. Provide opportunities for meaningful, spiritually fulfilling religious activities for each individual according to their own religious preference and background.
    2. Support individuals' right to express their religious affiliations and cultural traditions through preferred and relevant activities. Support diversity of individuals served through respect of religious preferences and/or cultural backgrounds.

    VII. Activities of Daily Living

    Provide assistance and training to residents in the following areas:

    1. Hygiene (shower/bathing, shaving, shampooing, toileting)
    2. Daily dressing and clothes selection
    3. Laundry and clothing care
    4. Daily housekeeping chores
    5. Eating (nutrition, meal preparation, table manners, etc).

    VIII. Recreation

    1. Plan, organize, and carry out diverse educational and recreational activities for all individuals.
    2. Integrate residents with non-disabled individuals of their own age group and with similar interests.
    3. Maximize independence so as to make recreational activities accessible without a car and without staff as often as possible.

    IX. Social

    1. Help residents develop necessary social skills, appropriate to life in their community.
    2. Provide simple, clear, nonjudgmental and supportive counseling and education, regarding sexuality and appropriate expressions of feelings.
    3. Train and encourage regular and socially acceptable use of the telephone, correspondence and email.
    4. Assist residents in maintaining ongoing family contact to whatever degree is positive for both the resident and his/her family.
    5. Maximize and facilitate peer relationships within the house.

    X. Communication

    1. Provide consistent follow-up in day-to-day interaction, to formal speech/communication therapy.
    2. Encourage and reinforce appropriate grammar, enunciation and sentence structure at all times to maximize each person's potential for verbal and written communication.

    XI. Safety

    1. Conduct regular, quarterly fire drills and follow-up training as needed for individuals who do not meet requirements for self-preservation.
    2. Provide formal and informal community independence assessments and household independence assessments and trainings as needed.

    XII. Programmatic

    1. Participate in and implement day-to-day formal and informal training goals outlined in each resident's ISP.
    2. Use and document task analysis forms where appropriate.
    3. Maintain daily health and/or behavioral program charts.
    4. Follow specific teaching plans for all targeted learning goals.
    5. Provide regular, ongoing written and oral reports on progress and problems with ISP training goals with all staff.

    XIII. Record Keeping

    1. Maintain careful, accurate petty cash records and checking account records of household expenditures which adhere to budgetary limits set by the Price Center.
    2. Provide careful informative daily log entries to facilitate staff communication and coordination.
    3. Maintain accurate medication and seizure records.
    4. Complete agency Incident and Accident/Injury Forms.

    XIV. Other

    1. Attend and participate actively in weekly staff meetings.
    2. Attend regular supervision sessions with supervisor.
    3. Assist in routine house and yard maintenance and cleaning.
    4. Participate in shopping responsibilities.
    5. Participate in regular program audits in preparation for DDS certification/licensing (QUEST) and CARF.
    6. Participate in formal training programs in a variety of topics related to job requirements.
