Job Summary:
The Licensed Practical Nurse (LPN) provides basic nursing care to residents under the direction of a Registered Nurse (RN) or physician. The LPN is responsible for monitoring patients' health, administering medications, providing wound care, documenting changes, and ensuring residents receive compassionate, high-quality care in accordance with nursing home policies and state regulations.

Key Responsibilities:
- Monitor residents' vital signs and general health condition
- Administer medications and treatments as prescribed
- Provide wound care and assist with personal hygiene tasks
- Observe and report any changes in residents' physical or emotional condition
- Maintain accurate and timely nursing documentation and medical records
- Collaborate with RNs, CNAs, physicians, and family members on patient care plans
- Support and supervise Certified Nursing Assistants (CNAs)
- Follow infection control, safety, and HIPAA protocols
- Assist during emergency situations and respond to resident calls promptly
- Participate in care plan meetings and resident assessments
- All other duties as assigned.
Qualifications:
- Licensure: Valid and active LPN license in Ohio
- Experience: Prior experience in long-term care or geriatric nursing preferred
- CPR Certification: Required
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate, patient, and respectful of elderly residents
Physical Requirements:
- Able to stand, walk, bend, lift (up to 50 lbs), and assist with transfers
- Ability to work in a fast-paced environment with frequent resident interaction
